OAS-CPP 6: Code of a form used by Health and Welfare Canada. A private trustee, appointed to receive Old Age Security and/or Canada Pension Plan payments on behalf of a beneficiary, is required to account for the benefits received and the expenditures made. The trustee must complete this form annually and at the termination of trusteeship.
